Finally took the time to get this recorded today, all measures by weight in grams.

350 g Mozzarella - Shredded room temperature.
20 g garlic - chopped.
80 g olive oil, not EVOO
40 g balsamic vinegar
5 g basil - dried.

Place mozzarella & garlic in the bowl of a food processor
Combine oil, vinegar and basil in a bowl and mix in to a paste, add to food processor
Pulse until well combined.

This will make a fairly loose spread, suitable for spreading on panini (That's what I use it for) for a stiffer, cracker spread, reduce oil to 40 g. May want to start lower on the balsamic and adjust to taste as well.
